Smith & Nephew has initiated a conflict-free mineral program, which includes working with their suppliers to gather data on the procurement of conflict minerals in their supply chain. Smith & Nephew has retained Claigan Environmental Inc. to collect this information on their behalf. I have attached a letter of authorization from Smith & Nephew and a blank Conflict Minerals Reporting Template 4.01a (CMRT). Please complete the attached CMRT v4.01a and return to the sender of this email by Monday, November 16, 2015.
If we do not receive your response, our records will indicate that you do not have a program in place and your status will be escalated to Smith & Nephew.
If your company does not use any tin, tantalum, tungsten, or gold in the materials, components, or services you supply to Smith & Nephew, let us know by replying directly to this email, and submit a completed CMRT.
Once a response is received the required attachments can be forwarded.
